Indiana Jones (at Disneyland) DOES have a restroom in the queue, you just need to let the CM before the stairs know you need it. It is right behind the elevators.
I think there would be a lot less resentment of people coming back after a bathroom break if you had to get some kind of pass or token from a CM before leaving and then rejoining the line. Most of the time, it’s impossible to tell the difference between someone rejoining after a pitstop and a garden-variety line cutter. Or, if they did something about the garden-variety line cutters, people would be more likely to assume that someone rejoining the line had actually been there before and had had to leave.
Before they had a bathroom in Avatar we got splattered with urine from someone who just releived themselves while waiting in an hour plus queue. Worst Disney experience ever.

It is better to provide enough attraction capacity to avoid having lines more than an hour long regularly.
